    In the volatile world of employee benefits, it is prudent to periodically review your company’s program to ensure its competitiveness. To have Heritage One review your current set of benefits and explore alternatives, please do the following three items:

    1. Click here and complete as much of the following form as possible.
    2. Provide claims experience for the past three years- month by month with the number enrolled for each coverage status (only if applicable). If you have this information, please e-mail it to jchalfant@heritage-one.com. If you do not have this information, do not worry. Go ahead and get us the rest of the information.
    3. Send us a census. You may do this one of two ways:
    A --You may provide this census information using this spreadsheet and email to jchalfant@heritage-one.com once it is completed, or

    B --Create your own census with the following information provided for each employee:
    • Name, Gender, Date of Birth or Age
    • Coverage Status (e.g. Employee only, Employee + Spouse, Employee + Child(ren), Family)- for Medical and Dental coverages only
    • Location- City, State and Zip Code (if there is more than one location)
    • Type of Plan (e.g. HMO, PPO) - only if offering more than one option
    • Income- only needed if seeking disability proposals or life insurance based on compensation.
    • Once you have completed the census, please e-mail it to jchalfant@heritage-one.com.
